Greater Manchester Connected Health City
GM CHC uses health data research to improve healthcare services in the North West.
Funded by the Department of Health, the Greater Manchester Connected Health City (GM CHC) Stroke project focuses on three aspects of the stroke care pathway:
1) Improving the recognition of stroke to maximise the proportion of acute stroke patients taken directly to a specialist stroke centre and minimising the number of none-stroke patients entering the stroke pathway.
2) Providing timely and focused referral to neurosurgery for Intracerebral Haemorrhage stroke patients.
3) Reducing secondary Stroke once patients have been discharged from hospital.
